Wheelchair-related physical restraints, lap belts, and other alternatives are intended to provide safe and adequate seating and mobility for individuals using wheelchairs. Physical restraints and lap belts are also helpful for positioning people in their wheelchairs to reduce the risk of injury during wheelchair tips and falls. However, when used improperly or in ways other than intended, injury or even death can result. Although widely prescribed, little evidence is available to direct professionals on the appropriate use of these restraints and lap belts and for whom these restraints are indicated. The purpose of this study was to conduct a review of available literature from 1966-2006 to identify the risks and benefits associated with lap belts while seated in wheelchairs. Twenty-five studies that met the inclusion criteria were reviewed. Nine studies reported the frequency of asphyxial deaths caused by physical restraints, nine studies reported the long-term complication and indirect adverse effects of physical restraints and lap-belt use, and seven studies reported the benefits of physical restraints and lap belts with individuals using power wheelchairs, www.easymedonline.com. BACKGROUND: handicap scooters and medicare power wheelchairs design parameters such as handrim diameter could affect propulsion. The purpose of this study was to examine the effect of handrim size (0.54, 0.43, and 0.32 m) on mechanical energy and power flow during wheelchair propulsion. METHODS: Twelve young normal male adults (mean age 23.5 years old) were recruited in this study. Both 3-D kinematic and kinetic data of the upper extremity were collected synchronously using a Hi-Res Expert Vision motion system and an instrumented wheel during wheelchair propulsion. FINDINGS: The kinetic, potential and total mechanical energy of the upper extremity increased as the handrim size increased. For each upper arm segment, the joint translational power and the rotational power of the proximal joint increased with increasing handrim size. The work done during a complete propulsion cycle with the larger handrim size is significantly larger than that using a smaller handrim (P<0.05). INTERPRETATION: The increased kinetic, potential and total mechanical energy were due to the increased linear velocity and the elevated positions of the upper extremity segments. The shoulder and trunk flexors increased the magnitude of their concentric contractions during propulsion with the large handrim as increased output power is required. By using mechanical energy and power flow analysis techniques, we evaluated the previously-reported effect of handrim size on mechanical cost and provided insight into the relationship between the two.

Peak PO and Formula: see text were significantly higher for SYNC (81.6+/-11.8 W versus 68.5+/-10.6 W; 26.4+/-4.5 ml kg(-1) min(-1) versus 21.2+/-3.0 ml kg(-1) min(-1)). At submaximal exercise levels, gross mechanical efficiency (ME) was significantly higher for SYNC (12.1+/-0.9% versus 9.7+/-1.4% at 41 W). No significant differences were found for PO (at equal velocity and slope), as derived from the SRM (SYNC and ASYNC), and from the drag test. DISCUSSION: The absence of any differences in PO between SYNC and ASYNC, and with respect to the drag test, rules out 'additional external work due to maintain the desired heading' in the ASYNC as an explanation for the lower performance in this mode. Lower peak performance and ME in ASYNC may be explained by the increased stabilizing muscle effort in the upper extremities and trunk in order to combine power production with stable steering. ASYNC is less efficient compared to SYNC. Similarly, peak performance capacity was higher for SYNC. CONCLUSION: External work does not differ between SYNC and ASYNC hand cycling. SRM readings appear valid for PO monitoring in hand cycling within the studied range of PO. SYNC is more efficient than ASYNC and leads to higher peak performance

Influence of hand cycling on physical capacity in the rehabilitation of persons with a spinal cord injury: a longitudinal cohort study

MAIN OUTCOME MEASURES: Peak oxygen uptake (Vo(2)peak) and peak power output (POpeak) determined in a handrim medical scooters peak exercise test, peak muscle strength of the upper extremities, and pulmonary function. RESULTS: A significantly larger increment in Vo(2)peak, POpeak, and elbow extension strength was found in subjects with paraplegia during clinical rehabilitation. No such effect was found in subjects with tetraplegia. In the postrehabilitation period, no influence of hand cycling on any outcome measure was found in subjects with paraplegia or subjects with tetraplegia. CONCLUSIONS: After correction for baseline values and confounders, regular hand cycling (once a week or more) appeared to be beneficial for improving aerobic physical capacity in persons with paraplegia during clinical rehabilitation. The small and heterogeneous study groups may have hampered the finding of positive results of hand cycling in persons with tetraplegia.

For example, because of the influence of others at work, I always believed that it was best for me to stay home 4 work nights each week and only be out late on a work night once a week. As a result there are many things I have not done because of that limiting belief that it was “not good for me to go out too many times during the week”. One of the things I missed out on was learning Spanish, something I have wanted to do for years.
So why is it that we should be making conscious choices about which information we take in so that we are Continuously Learning?

I have just completed a learning event with Live Out Loud and Loral Langemeier in Lake Tahoe. Believe me, it was a stretch and really assisted me in getting outside of my comfort zone. My life has changed for the better because of that experience and I am preparing myself for the future!
The events that have happened in my life where I have had to get out of my comfort zone have all in some way, big or small, improved my life. There are still times where I want to wallow in my comfort zone. However, now I know the perils of staying there, so I do not stay there very long.
When I first started on my journey of building my home business, our dream was to be able to sponsor 10 children with “The Plan”. As we have learned and grown through the process our focus is now to sponsor 100 children. The learning has changed our attitude about life and money, which of course has resulted in changing our lives. The focus of our life has changed for the better.

Learning assists us in adapting to change. Think of a time when you dreaded having to learn something new. Why were you dreading it? Was it because you are not in the habit of learning? Did you believe it would be too hard? Did you not see any benefit for the learning? Getting back into the habit of learning is just like starting out on a bicycle, it takes a lot more effort to get going than it does once you have attained the desired speed. When we are learning we are involved with life, we are out with our peers or a whole new group of people.
What are new information are you feeding yourself? I realized that the fiction books I was feeding myself were simply a form of escape. Very similar to the young boy we met on the streets of Le Ciba in Honduras. He was one of the kids who were abandoned to the streets as young as the age of two. What he did to escape the reality of his own life was to sniff gasoline. After meeting him I realized that I was doing the same thing by reading too many fiction books.
After that I started feeding myself with more positive material. If this is one of your escape mechanisms and you desire to change it, you may want to switch the type of books that you read. Biographies are a great way to switch from fiction because they are stories too.
Once we have begun to change our learning habits by making conscious decisions about the information we feed ourselves on, the next step is to teach what we have learned. Teaching others is the best way to learn it even better. One of my great desires is for my family and friends to learn how not to get left behind. That is the reason I choose to write these articles, because assisting you in your learning also assists me in understanding better myself.
